Service delivery to our clients is our first priority! As a Mental Health Practitioner - Support Coordinator you will be responsible for providing recovery focused mental health services, customer wellbeing and ensure our clients and their families have the support and services they need to achieve their goals. Knowledge of the Fee for Service NDIS model is essential.

- Support customer wellbeing by providing personalised support coordination services
- Assist customers to define their goals and aspirations identified in NDIS plans.
- Undertake necessary assessments in line with the customers funding, need and choices.
- Provide information, advice, support and mentoring to families and carers to assist them to access services relevant to their health and social needs
- Facilitate the re-engagement and maintenance of family and carer relationships
- Passionate Mental Health Practitioners
- Team players - our common goal is our customers!
- Proven experience in developing strength based assessments
- The ability to build relationships with customers and their family and carers
- Knowledge and experience of the Fee for Service (NDIS) model
- Tertiary qualifications (minimum certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health related field
