Community and Preventive Health (CPH) is a diverse group of service units delivering care and interventions outside the acute hospital setting in most cases. These services comprise all aspects of the care and life continuum including population health, prevention, early detection, management, reducing disease progression and complications, Aboriginal and Torres Strait Islander health care, rehabilitation and end of life care. Services are delivered in a variety of settings such as residential aged care facilities, community-based centres, satellite and outreach services, mobile vans, hospitals and in people's homes.

Key Responsibilities include:

• Assume responsibility and accountability for own actions, standards, and the outcomes of own nursing practice as well as for activities delegated to other nurses and unregulated healthcare workers.
• Contribute to positive patient outcomes by;
• providing supervision, coordination and leadership to effectively assess, plan, manage and evaluate patient outcomes at an advanced practitioner level particularly for patients with unpredictable outcomes
• using evidence based clinical practice that maintains patient safety
• leading management and coordination of more complex patients
• facilitating accurate and timely communication to drive effective multidisciplinary team functioning
• role modelling best practice in clinical and professional domains.

Remuneration based pro rata on equivalent full time value up to $115,508 p.a. comprising salary rates: $47.68 - $51.05 p.h. employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (Nurse Grade 6 (1)) (temporary part time position available until 31/10/2021, 45.6 hrs p.f.).
