PERSOLKELLY is currently recruiting on behalf of an established In-Home Healthcare services provider who has been trading for over 30 years in the market and currently has a requirement for a Palliative Care Registered Nurse to join their team wo. This is full time 38 hours per week.

Duties & Responsiblities:

  • Drawing on your experience as an RN to deliver the best practice holistic clinical palliative care within the community.
  • You will be the expert in providing patient centered, evidence based specialist services for patients with palliative and end of life care needs making the patient feel safe and comfortable.
  • Liaising and supporting the patients support network during end of life care may include recommending and referring multi disciplinary teams and organisations as required.
  • Working within the community.
  • Reporting directly to the Clinical Services manager
  • Working autonomously as well as coordinating with a collaborative team whilst working closely with our Palliative Care Specialist to develop this new program.
  • A pivotal role in the development of the clinical program
  • Coordinating all aspects of patient care from assessment and care planning to providing direct patient care
  • Liaising with hospitals, doctors and family members to provide safe palliative care to patients in their own home
  • Managing the logistics of palliative home care services, including medication and stock requirements

Requirements:

  • Current unrestricted AHPRA Registration as an Registered Nurse
  • Palliative care qualifications along with minimum 5 years' experience in palliative care
  • Clinical knowledge, skills and experience in acute care
  • An expert in providing patient centred, evidence-based specialist services for patients with palliative and end of life care needs
