  • Total Indicative Remuneration: $123,346 to $128,860 (p.a/pro rata) - Multiple positions F/T P/T, hours negotiable - Temp up to 28 March 2021 - RN3

Provide clinical nursing expertise for specified individual patients/clients and/or groups/populations, and/or provide clinical leadership for an area of practice and may work in a variety of clinical settings. Employees classified at this level are experts within their area and stream of practice and have a significant degree of autonomy and decision making.

With the support of Southern Adelaide Local Health Network (SALHN) and Priority Care Centre (PCC) partners, this position will lead the operationalisation of the PCC strategy for SALHN.

The incumbent is to maintain contemporary knowledge about safe clinical triage, hospital avoidance strategies and community alternatives for delivery of clinical care. Through collaboration with the PCC, SA Ambulance Service (SAAS), SALHN Emergency Departments (ED) and community partners the role coordinates and facilitates safe patient diversion from the ED and/or via SAAS to prevent unnecessary SALHN presentations and admissions by coordinating and/or delivering care within the PCC.

Various practice models may be used to enact this role, including but not limited to:

  • Act as the liaison between SAAS and the PCC to coordinate the diversion and flow of appropriate ambulance patients to the PCC
  • Act as the liaison between the PCC and the ED to coordinate the flow of walk-in patients from ED to the PCC
  • Lead the data collection, monitoring, analysis, measurement and evaluation of clinical practice to meet requirements of PCC strategy
  • Coordinate, support and deliver safe, quality health care to patients within the PCC
  • Coordinate patient access to community delivered services as required supporting continuation of care, preventing unnecessary ED presentations
  • Monitor the SA Health consumables provided to support treatment and care
  • Provide support and education to the PCC staff to build capabilities and quality improvements
  • Contribute to the assessment/triage and coordination of deteriorating patients through coordination back to specialist rapid assessment clinics and or direct admission to hospital
  • Provide treatment to patients under the supervision of the PCC GP

In this role you will accept accountability for nursing practice, the outcomes of nursing practices for the specific patient/client group, the professional advice given, delegations of care made and for addressing inconsistencies between practice and policy.

For appointment you must be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
