Working in a multidisciplinary community care team, you will work in a restorative care approach to return clients to their usual activities of daily living. Independently managing a diverse caseload, you will provide assessment and intervention for home safety, home modifications, and equipment prescription. You may be required to work as part of the specialist Transitional Care and Rehabilitation team working across the Riverland Mallee Coorong Local Health Network (LHN), focusing on improving health outcomes, functional capacity and quality of life in accordance with the State-Wide Rehabilitation Service Plan 2009-2016 and the objectives of the Councils of Australian Government’s (COAG) Sub-Acute Services Initiative. As part of your role, you may also contribute to the clinical supervision of less experienced Allied Health Professionals, Allied Health Assistants and students. Riverland Community Health Service provides therapeutic nursing and allied health professional services, specialised assessment, support services, health promotion and advice on health issues. Our staff consult in the five major Riverland towns of Berri, Barmera, Loxton, Renmark and Waikerie. You must hold an appropriate degree or equivalent qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of SA (up to July 2012), or the Occupational Therapy Board of Australia (post July 2012). With experience in providing Occupational Therapy care, you will be skilled in providing a broad range of clinical and consultative services across a range of service settings, including one-on-one, group based and health promotion activities. This will include skills in supporting clients, their carers and families across the Patient Journey, providing effective assessment and triage, timely referrals, accurate information, coordinated care and prompt follow-up.
